We are a local, doctor owned, licensed home healthcare company whose mission is to help individuals remain in their homes with integrity, dignity, and satisfaction and provide supportive guidance for disease
management.
We offer primarily three services to assist in meeting our clients’ needs: Care Management, Companion Services, and Skilled Healthcare Services. However, Care Management is central to all that we do at CHC Services. Our clients can receive our services in various combinations, including:

Care Management Services
Care Managers are experienced nurses and clinicians that help individuals and families manage the complexities of a new or chronic illness or disability. Clients can receive Care Management as an integral component of a comprehensive set of services.

Companion Services
Companion non-medical services are designed to help an individual meet their everyday needs in a safe and effective manner. Such services are provided by our friendly, compassionate, qualified, and responsible Care Specialists and are always provided in the context of a well-defined Care Plan with medical/nursing oversight. Care Specialist services include assistance with all personal care, bathing, cooking, housekeeping and companionship.

Skilled Healthcare Services
Skilled healthcare services such as nutrition counseling, therapy services including physical, occupational, speech, recreational, and more are also available through CHC Services. And, like Companion Services, they are usually provided within the context of a Care Plan.

Other Services
We offer a variety of other services from financial management to technology including telehealth and biometic services and equipment to support independence and safety in the individual’s living environment. The MD2 personal medication system is one such product that promotes independence and safety with medication management. It has built in reminding, monitoring, and will notify up to four family members, friends, and/or care managers by telephone if medications are missed. Call for more information 425-275-5858.
